Dear Doctor N:
What is the main difference between the prescriptions Prempro and Premarin? Would one be of more benefit than the other and/or safer to a woman with a complete hysterectomy, for treating hot flashes and vaginal dryness?

Premarin is a form of estrogen, Prempro is Premarin and Provera, a progestin. In general, there is no reason to use progestins when one does not have a uterus unless you are known to have endometriosis that remains in the pelvis or abdomen.
Progestins taken on a daily basis may cause symptoms of irritability, depression, breast tenderness and swelling. In addition, they may interfere with some of the benefits of estrogen.
